PeaceHealth is seeking a Pharmacist for a Full Time, 1.00 FTE, Variable position. The general salary range for this job opening at PeaceHealth is $64.78 – $97.18. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.

This position is responsible for performing all necessary functions to ensure optimal use of medications to achieve specific outcomes that improve a patient's quality of life. Functions included in providing compassionate pharmaceutical care are both cognitive and medication distribution related and include: dispensing and distributing drugs; advising physicians and patients regarding proper usage of prescribed drugs; collaboration with other healthcare professional to plan, monitor, review and evaluate patient effectiveness; monitoring patients for side effects and drug interactions.

DETAILS OF POSITION:

  • Reviews medication orders for appropriateness and accuracy including, but not limited to: contraindications, allergies, dose, route, frequency, duplicate therapy and formulary status.

  • Interprets, clarifies, and rectifies physician medication and IV orders, including the medication reconciliation process.

  • Interprets, clarifies, and rectifies prescriptions and selects medications for outpatient dispensing.

  • Monitors all patients on specified target drugs and initiates pharmacokinetic dosing protocols.

  • Evaluates drug therapy for appropriateness and makes drug therapy recommendations to clinician.

  • Prescribes drug therapy per collaborative drug therapy management agreements/Medical Executive Committee approved protocols.

  • Dispenses the final product for administration and performs patient counseling.

  • Detect, monitor, and report adverse drug events and medication errors (EIR).

  • Supervise and train pharmacy technicians to assure accurate order entry, preparation, and proper technique.

  • Supervise and trains pharmacy interns and externs. May serve as preceptor for the intern or extern as defined by the State Board of Pharmacy.

  • Participate in development of drug protocols and clinical pathways.

  • Perform drug usage evaluations on selected drugs and reports results to the Pharmacy & Therapeutics Committee. Completes quality assurance auditing as required for prescriptive authority.

  • Performs other duties as assigned.

WHAT YOU BRING:

  • Bachelor's Degree Required: Pharmacy or

  • Doctor of Pharmacy Required: from an accredited school

  • Required: Demonstrated basic pharmacy clinical skills and

  • Required: Demonstrated customer services skills.

  • Required: Pharmacist - Oregon currently licensed or able to obtain license and

  • Preferred: Basic Life Support and

  • Preferred: Advanced Cardiac Life Support and

  • Preferred: Pediatric Advanced Life Support

Skills

  • Basic computer and keyboarding skills including MS Office applications (Required)

  • Ability to identify medications and to read graduation marks on syringes and flasks (Required)

  • Strong planning, prioritizing and organizational skills (Required)

  • Excellent communication skills both verbal and written (Required)

  • Ability to think and act independently (Required)

  • Strong mathematical and analytical skills (Required)

Oregon West Network-Home Infusion Additional Essential Functions:

  • Evaluates, manages, and coordinates the use of infusion pumps and devices.

  • Assures that sterile products are prepared under conditions defined by the US Pharmacopoeia and State and Federal law.

  • Participates in activities necessary for compliance with the Joint Commission’s Accreditation Standards for Home Care.

Oregon West Network - Clinic:

  • May require Certified Geriatric Pharmacist.

  • May require Diabetes education.

Working Conditions

Lifting

  • Ability to move around area with occasional sitting.

  • Occasional squatting/ kneeling/ bending.

  • Fine motor skills to be able to grasp to perform precise procedures frequently.

  • Frequent reaching.

  • Lift/Carry: Occasionally up to 30 lbs.

  • Push/Pull: Occasionally up to 14 lbs. force.

Environmental Conditions

  • Exposure to biohazard, body fluids and airborne particles.

  • Must be able to complete tasks in a noisy environment.

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

  • Ability to communicate and exchange accurate information.
